Upon initial contact we will provide an assessment of both the client and their home. The assessment is very important in determaning the needs and safety of the client. We also introduce caregivers to the client or family member to recommend a perfect match of personality and experience.
After the assessment we provide our home care aides with a care plan which describes the client’s health and physical limitations. We establish a long and short-term goal that each member of the Pathfinder Health Care team must use to either reach their goal or assist in recuperation from surgery or illness.
